The Missouri state backflow test report (form 780-0804) and WaterOne's test report are filled from the saved test, on their own layouts. KC Water, the BPU in Kansas City, Kansas, and utilities that take reports through an online portal get a work sheet of the fields they ask for, marked as not their form, so the tester can enter it where the utility wants it. Each issued report keeps the form edition it was issued on, so a later form revision never rewrites one you already filed.

No. The readings are the tester's own measurements and pass or fail is the tester's own call, as every form requires. A failed test keeps its readings, each repair lists the component and the parts used, and the final test after repair sits beside the initial one. The next due date comes from the passing test, and the backflow due list shows every assembly coming up.

Water heaters, boilers, softeners, well pumps and sump pumps each offer the readings that equipment takes: water pressure, gas pressures, flue temperature and carbon monoxide on a heater, supply and return temperatures on a boiler, hardness in and out on a softener. Findings are numbered, each repair names the findings it fixes, and each verification names the repair it proves. The customer's report follows the same order.

Yes. Register the line, then record observations with PACP defect codes, the grades your certified inspector assigns, distances, and a linked or uploaded video. Observations and locates export as a spreadsheet. It is not a NASSCO exchange database, so a city that requires one still needs it from NASSCO-certified software.
